• cadastral region
• original owner (earl, landlord, etc.)
• name and surname of person to whom allotment was granted
• name and maiden name of wife of person to whom allotment was granted
• year of allotment execution
• register numbers of plots of ground
• request for search of graphic part of allotment plan (map)
• reason of request
• relationship to person to whom allotment was granted, or verified authorisation in the case of representing such person
• the address to which the settlement of request should be delivered.

Verified copies of allotment registers and allotment deeds are charged according to tariffs of administrative fees specified in the supplement of  Act of Slovak National Council No 232/1999 Coll., by which Act No 145/1995 Coll. on Administrative Fees is changed and accomplished in compliance with later regulations by the amount of 50.- Sk (Slovak koruna) for copy of 1 page and by 50.- Sk for verifying of 1 page.

A copy made from map documentation is charged by administrative fee of 250.- Sk for 1 page of A4 format and by 50.- Sk for verifying of 1 page according to Act of Slovak National Council No 468/2000 Coll. on Administrative Fees.

If the result of map documentation search is negative, search activity is charged by the amount of 100,- Sk for request according to article 14 paragraph 3 of Regulation No SVS-204-2001/00016 of Archives Administration of the Ministry of the Interior of the SR.

Notary and judicial commissioner entrusted by execution of patrimonial affairs are obliged to pay administrative fees according to above mentioned Acts on the basis of Regulation No 5698/2000-74 of the Ministry of Finance of the SR.

Representatives of geodetic offices participating in creation of renewed land registers are obliged to pay administrative fees according to above mentioned Acts on the basis of Regulations of Archives Administration of the Ministry of the Interior of the SR No SVS-204-98/00410 and No SVS-204-99/00052.
