The request must contain the following information:
- name and surname
- date and place of birth
- name of employer
- period of employment
- reason for certificate issue
Administrative fees:
Certificates issued for retirement pension and health insurance purposes are free of charge
( § 4 art. 2, Act of National Council SR No 232/1999 Coll. on Administrative Fees
).
